our first visit to Thrive Psychiatry LLC establishes a vital foundation for our relationship with you. During your first visit, we make sure to obtain important background information about you and give you time to get to know your provider.

Most appointments at Thrive will be done through telemed. On Tuesdays in-person appointments are available. ADHD and Substance Abuse evaluations must be done in person. Follow-up appointments can be done through telemed. Telemed appointments can be done on a phone, tablet, or computer.


In-person appointments - Tuesdays , 9 am - 5 pm.

Patient Forms

After your appointment has been confirmed, a link to establish your Patient Portal will be sent to your email. Please create your account and complete the intake forms. While no one loves to fill out forms, these need to be done in advance. This allows me to review your information before the appointment. Also, it may be necessary to gather information from teachers or others, and having all of that ready at the evaluation makes our time much more productive.

What to Expect

Being well-prepared for your appointment will ensure that I have all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Take some time to review the Meet Me page to familiarize yourself. If your appointment is in-person, please wait in the lobby and I will come out to get you. If your appointment is telemed, please log in to your portal and join the appointment ahead of your scheduled time. I look forward to your first visit.
